Job Summary

Trident Lake Health is seeking a Registered Nurse (RN) to join our growing team in the Phoenix metro area (East and West Valley). If you are passionate about supporting adults and providing compassionate, person-centered care, we’d love to connect.

We provide home and community-based services to adults across Arizona. Our multidisciplinary team collaborates to deliver individualized, evidence-based care that meets each patient/client where they are.

Pay

Pay: $50.00 per follow-up visit

Responsibilities

  • Complete skilled nursing follow-up visits, including ongoing assessments (start-of-care/OASIS visits are completed by the agency).
  • Implement and monitor individualized nursing care plans.
  • Perform skilled nursing services within the RN scope of practice.
  • Monitor patient/client health status and communicate changes to physicians and the interdisciplinary team.
  • Provide education to patient/client, families, and caregivers regarding disease management, medications, and health promotion.
  • Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
  • Ensure compliance with CMS home health requirements, AHCCCS, HIPAA, and company policies.
  • Complete timely and accurate clinical documentation.

Requirements

  • Active Arizona Registered Nurse (RN) license in good standing.
  • Arizona Level One Fingerprint Clearance Card.
  • Current CPR/BLS certification.
  • Valid Arizona driver's license and reliable transportation.
  • Current professional liability (malpractice) insurance (required for 1099 contractors).
  • Current automobile insurance.
  • Welcome to apply if no experience in home-based care.
  • Strong communication and collaboration skills.
