Job Summary

Trident Lake Abilities Care is seeking a PRN Pediatric Physical Therapist Assistant to provide treatment sessions to children in their homes and via telehealth across Arizona, under the supervision of a licensed Physical Therapist.

We hire by zip code, so you will never drive more than 30 minutes to an in-home visit.

We provide in-home and telehealth therapy to children with developmental disabilities who are members of the Arizona Division of Developmental Disabilities (DDD). Our team partners with families to deliver play-based, evidence-based care that meets each child where they are.

Pay (per visit)

Pay: $48.00 – $58.00 per visit

Treatment session, in-home (60 min): $58

Treatment session, telehealth (60 min): $48

Group session: +$10 for each additional child

Sessions shorter than 60 minutes are prorated

Responsibilities

Provide PT treatment sessions following the plan of care set by the supervising Physical Therapist

Help children build gross motor skills, balance, strength, mobility, and gait through play-based activities

Coach families on home exercise programs and safe handling

Report progress and changes to the supervising Physical Therapist

Communicate with families and the care team

Complete accurate, timely documentation that meets DDD/AHCCCS requirements

Use Electronic Visit Verification (EVV) at every in-home visit

Requirements

Associate degree from an accredited PTA program

Active Arizona Physical Therapist Assistant certification

Arizona Level One Fingerprint Clearance Card (or ability to obtain before start)

Current CPR/First Aid certification

DDD Article 9 (Managing Inappropriate Behaviors) training, or willingness to complete before start

Valid Arizona driver's license, reliable transportation, and auto insurance

Current professional liability (malpractice) insurance

Reliable internet and a private, quiet space for telehealth sessions

Comfortable using an EMR and mobile EVV check-in/check-out

Pediatric experience preferred; new grads welcome

Contract Terms

1099 independent contractor, PRN; you set your availability

Paid per completed visit; no mileage reimbursement

Must carry your own professional liability (malpractice) insurance

Hired by zip code: you are only assigned in-home visits within a 30-minute drive of your home base

Telehealth visits available statewide

After-school hours are in high demand
