Job Summary

Trident Lake Abilities Care is seeking a PRN Pediatric Occupational Therapist to evaluate and treat children in their homes and via telehealth across Arizona. You will complete evaluations, build each child’s plan of care, and supervise our COTAs, who help carry out treatment.

We hire by zip code, so you will never drive more than 30 minutes to an in-home visit.

We provide in-home and telehealth therapy to children with developmental disabilities who are members of the Arizona Division of Developmental Disabilities (DDD). Our team partners with families to deliver play-based, evidence-based care that meets each child where they are.

Pay (per visit)

Pay: $65.00 – $115.00 per visit

Evaluation (in-home or telehealth): $115

Treatment session, in-home (60 min): $85

Treatment session, telehealth (60 min): $65

Group session: +$10 for each additional child

Sessions shorter than 60 minutes are prorated

Responsibilities

Complete pediatric OT evaluations and re-evaluations

Treat fine motor, sensory processing, self-care (ADLs), visual-motor, and feeding skills

Develop family-centered plans of care and home programs

Recommend adaptive equipment and environmental modifications

Supervise and collaborate with COTAs assigned to your caseload

Communicate with families, DDD support coordinators, and the care team

Complete accurate, timely documentation that meets DDD/AHCCCS requirements

Use Electronic Visit Verification (EVV) at every in-home visit

Requirements

Active Arizona Occupational Therapist license

Arizona Level One Fingerprint Clearance Card (or ability to obtain before start)

Current CPR/First Aid certification

DDD Article 9 (Managing Inappropriate Behaviors) training, or willingness to complete before start

Valid Arizona driver's license, reliable transportation, and auto insurance

Current professional liability (malpractice) insurance

Reliable internet and a private, quiet space for telehealth sessions

Comfortable using an EMR and mobile EVV check-in/check-out

Pediatric or sensory experience preferred; new grads with pediatric interest welcome

Contract Terms

1099 independent contractor, PRN; you set your availability

Paid per completed visit; no mileage reimbursement

Must carry your own professional liability (malpractice) insurance

Hired by zip code: you are only assigned in-home visits within a 30-minute drive of your home base

Telehealth visits available statewide

After-school hours are in high demand
