Job Summary

Trident Lake Health is seeking an Occupational Therapy Assistant (OTA) to join our growing team in the Phoenix metro area (East and West Valley). If you are passionate about supporting adults and providing compassionate, person-centered care, we’d love to connect.

We provide home and community-based services to adults across Arizona. Our multidisciplinary team collaborates to deliver individualized, evidence-based care that meets each patient/client where they are.

Pay

Pay: $55.00 per follow-up visit

Responsibilities

  • Provide occupational therapy interventions under the direction and supervision of a licensed Occupational Therapist
  • Implement individualized treatment plans developed by the supervising Occupational Therapist
  • Assist patient/client in improving fine motor skills, sensory processing, activities of daily living (ADLs), and functional independence
  • Monitor progress and communicate changes to the supervising Occupational Therapist
  • Collaborate with patient/client, families, and the care team
  • Complete accurate, timely visit documentation

Requirements

  • Active Arizona Occupational Therapy Assistant license
  • Arizona Level One Fingerprint Clearance Card
  • Current CPR/BLS certification
  • Valid Arizona driver's license and reliable transportation
  • Current professional liability (malpractice) insurance (required for 1099 contractors).
