Job Description

Job Summary

Trident Lake Health is seeking an Occupational Therapist (OT) to join our growing team in the Phoenix metro area (East and West Valley). If you are passionate about supporting adults and providing compassionate, person-centered care, we’d love to connect. This is a PRN, evaluation-focused role: you complete evaluations, 30-day reassessments, recertifications, and discharges, and our licensed COTAs complete follow-up treatment visits under your plan of care.

We provide home and community-based services to adults across Arizona. Our multidisciplinary team collaborates to deliver individualized, evidence-based care that meets each patient/client where they are.

Pay

Pay: $75.00 – $100.00 per visit, tiered: evaluation $100; recertification $95; 30-day reassessment or discharge $90; follow-up visit (when assigned) $75

Responsibilities

  • Complete OT evaluations, 30-day reassessments, recertifications, and discharges (start-of-care/OASIS visits are completed by the agency)
  • Develop individualized plans of care targeting ADLs, IADLs, fine motor skills, cognition, home safety, and adaptive equipment
  • Supervise and collaborate with the COTAs who complete follow-up treatment visits, and adjust the plan of care as needed
  • Collaborate with patient/client, families, and the care team
  • Complete accurate, timely visit documentation

Requirements

  • Active Arizona Occupational Therapist license in good standing.
  • Current CPR/BLS certification.
  • Arizona Level One Fingerprint Clearance Card (or ability to obtain prior to start).
  • Valid Arizona driver's license.
  • Current professional liability (malpractice) insurance (required for 1099 contractors).
  • Welcome to apply if no experience in home-based care.
  • Ability to work independently while maintaining effective communication with the interdisciplinary team.
  • Strong clinical reasoning, organizational, and time management skills.
  • Excellent written and verbal communication skills.
  • Ability to complete documentation accurately and within required timelines.

Preferred Qualifications

  • Experience working with adults in home-based or community settings.
  • Experience in home health, outpatient, or community-based rehabilitation.

Benefits

  • Flexible PRN schedule.
  • Tiered per-visit pay that reflects visit complexity.
  • Collaborative and supportive interdisciplinary team environment.
