Job Summary

Trident Lake Health is seeking a Licensed Practical Nurse (LPN) to join our growing team in the Phoenix metro area (East and West Valley). If you are passionate about supporting adults and providing compassionate, person-centered care, we’d love to connect.

We provide home and community-based services to adults across Arizona. Our multidisciplinary team collaborates to deliver individualized, evidence-based care that meets each patient/client where they are.

Pay

Pay: $40.00 per follow-up visit

Responsibilities

  • Provide skilled nursing care under the direction and supervision of a Registered Nurse or physician within the LPN scope of practice.
  • Monitor patient/client health status and report changes to the supervising Registered Nurse and healthcare team.
  • Administer medications and treatments as prescribed.
  • Assist with health assessments and implementation of individualized care plans.
  • Educate patient/client, families, and caregivers on medications, treatments, and health maintenance.
  • Collaborate with patient/client, families, and the care team.
  • Complete accurate, timely visit documentation.

Requirements

  • Active Arizona Licensed Practical Nurse (LPN) license.
  • Arizona Level One Fingerprint Clearance Card.
  • Current CPR/BLS certification.
  • Valid Arizona driver's license and reliable transportation.
  • Current professional liability (malpractice) insurance (required for 1099 contractors).
  • Current automobile insurance.
