Job Overview

Trident Lake Health is seeking a Physical Therapist Assistant (PTA) to join our growing team in the Phoenix metro area (East and West Valley). If you are passionate about supporting adults and providing compassionate, person-centered care, we’d love to connect.

We provide home and community-based services to adults across Arizona. Our multidisciplinary team collaborates to deliver individualized, evidence-based care that meets each patient/client where they are.

Pay

Pay: $55.00 per follow-up visit

Responsibilities

  • Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist
  • Implement individualized treatment plans developed by the supervising Physical Therapist
  • Assist patient/client in improving mobility, balance, coordination, strength, endurance, gait, and functional independence
  • Educate patient/client and caregivers on home exercise programs and safe mobility techniques
  • Monitor patient/client progress and report changes to the supervising Physical Therapist
  • Collaborate with patient/client, families, and the care team
  • Complete accurate, timely visit documentation
  • Maintain compliance with CMS home health requirements, AHCCCS, HIPAA, and company policies.

Requirements

  • Associate degree from an accredited Physical Therapist Assistant program
  • Active Arizona Physical Therapist Assistant license
  • Arizona Level One Fingerprint Clearance Card
  • Current CPR/BLS certification
  • Valid Arizona driver's license and reliable transportation
  • Current professional liability (malpractice) insurance (required for 1099 contractors).
  • Experience or interest in home-based care
  • Strong communication and collaboration skills
